We do ask for payment (less any applicable insurance or adjustments) at the time of service. We accept:
- Checks from established patients with valid identification
- Visa, Mastercard and Discover
- CareCredit *
*Outside financing for either a revolving line of credit and interest free periods or a fixed rate monthly payment option.
We offer Senior Citizen patients ages 65 and older a 5% adjustment.
We will process any financial assistance in the form of dental insurance. As a courtesy, our office accepts assignment of benefits (payment on your behalf from the insurance company) in circumstances that your policy allows us to. This minimizes your role in the sometimes complicated process of filing claims to obtain reimbursement. It also reduces your patient payment due when services are rendered. In most circumstances we are able to verify your coverage quickly and easily, however that is not always the case with smaller carriers. Because of this, we ask that you provide us with the following important information at least 1 business day prior to your appointment:
¨ Subscriber’s Name
¨ Subscriber’s Social Security Number and Member ID number
¨ Name of Subscriber’s Employer
¨ Group Plan Number
¨ Insurance Carrier Name, Address, and Phone Number.
In the event that you are not able to provide us the information before your appointment, you may be responsible for payment at the time of service. As soon as your benefits are verified, we will process all necessary forms for you to receive reimbursement.
